Assistant/Nurse Practitioner - Primary Care Location: New Windsor,
NY Job Type: Full time Permanent Shift: Provider may select a 4 or
5 (regular length) day week; flexible scheduling and work-life
balance. Salary: $120,000 to $130,000 Annually $5K productivity &
quality bonus potential Full-time Benefits PRIMARY JOB DUTIES The
duties listed below represent typical responsibilities of the
position; not all duties may be assigned at all times. Clinical
Care Provide comprehensive primary care services including
preventive care, routine wellness visits, acute illness evaluation,
and chronic disease management. Perform complete and focused
histories and physical examinations using evidence-based guidelines
and best practices. Diagnose and manage common primary care
conditions and coordinate care for complex patients in
collaboration with physicians and care team members. Prescribe
medications, therapies, and treatments within scope of practice;
maintain accurate and up-to-date medication lists. Order,
interpret, and follow up on diagnostic tests, labs, and imaging
appropriate to primary care. Develop, implement, and monitor
individualized care plans and treatment goals for patients.
Emphasize preventive care, patient education, lifestyle
modification, and patient engagement in ongoing health management.
Patient Communication & Care Coordination Provide education and
counselling to patients and families regarding diagnoses, treatment
plans, medications, and preventive care. Collaborate closely with
physicians, nurses, medical assistants, care coordinators, and
other team members. Participate in daily huddles and
interdisciplinary care planning. Coordinate referrals and follow-up
care as needed. Documentation & Quality Document all patient
encounters thoroughly and accurately in the electronic medical
record (EMR). Utilize population health management tools to support
quality metrics, preventive care compliance, and chronic disease
outcomes. Follow organizational policies, clinical guidelines, and
appropriate diagnosis and billing documentation standards.
Professional Practice Maintain current knowledge of primary care
clinical guidelines and best practices. Participate in continuing
medical education (CME) relevant to primary care. Participate in
provider meetings, trainings, and quality improvement initiatives.
Work within a patient-centered medical home and team-based care
model. Perform related duties as assigned. Qualifications: PA or NP
degree from an accredited school Board certified in Family Medicine
or Internal Medicine Valid and unrestricted NY state license Valid
DEA license (or ability to obtain) Current BLS/ACLS certification
