compassionate and dedicated Nurse Practitioner to join our care
team at Laconia Nursing Home, a well-established skilled nursing
facility in the Bronx. This full-time position offers a consistent
schedule and a supportive environment focused on quality resident
care. Responsibilities: Provide on-site primary and acute care to
both long-term and subacute residents Perform daily rounds,
diagnose and manage conditions, and develop care plans Work closely
with nursing staff and interdisciplinary teams to coordinate care
Document services, communicate with families, and support care
transitions Help maintain high clinical standards and regulatory
compliance Requirements: Active Nurse Practitioner license in New
York Minimum 6 months NP experience (ICU or SNF background
preferred) Strong clinical decision-making and communication skills
Commitment to geriatric and post-acute care Ability to work
