RN will perform both physical and psychosocial assessments of patients in need of hospice or palliative care. As the RNCM, you will electronically document visit occurrences. Patient visits will be made in facility settings only - FREE onsite parking is availble at Skilled Nursing facility You will develop and implement nursing Goals of care and make recommendations for care based on terminal patients' needs.
