Job Description Job Description Nurse Practitioner – Nursing Home Location: Bronx, NY Position Type: Full-Time | Permanent Salary Range: $135,000 – $150,000 (based on experience) Position Overview We are seeking a motivated and compassionate Nurse Practitioner (NP) to provide primary and follow-up care to residents in long-term care and subacute nursing facilities across NYC. This is an excellent opportunity for both experienced providers and new graduates seeking to grow in a supportive, patient-focused environment. The NP will conduct routine assessments, manage chronic conditions, coordinate care with facility staff, and collaborate closely with supervising physicians and interdisciplinary teams to ensure high-quality patient outcomes. Key Responsibilities Perform comprehensive physical assessments and routine follow-up visits Diagnose and treat acute and chronic medical conditions Develop and implement individualized treatment plans Order and interpret laboratory tests and diagnostic imaging Prescribe medications and administer treatments as appropriate Document patient history, examinations, assessments, and care plans accurately in EMR Coordinate care with nursing home staff and collaborating physicians Provide patient and family education regarding treatment plans and preventive care Administer vaccinations and promote disease prevention strategies Refer patients to specialists within the network when necessary Qualifications Master’s Degree in Nurse Practitioner Program (Family or Adult Geriatrics preferred) Active New York State Nurse Practitioner license National Board Certification (AANP or ANCC required) Bilingual Spanish is a plus EMR/EHR experience preferred New graduates are encouraged to apply Work Environment Nursing home and long-term care facility setting Collaborative, multidisciplinary care model Supportive team environment with strong physician collaboration Company Description LanceSoft is rated as one of the largest staffing firms in the US by SIA.
