Specified Experience: Two (2) years in a home setting within the
last ten (10) years License: Current RN Licensure in New York State
Certifications: BLS required before start date Must-Have: - Two (2)
years of experience in a home setting within the last ten (10)
years. - Recent experience conducting comprehensive assessments of
patients. - Experience working in the field for a Certified Home
Health Agency, Licensed Home Care Service Agency, or Managed
Long-Term Care. - Experience conducting assessments similar to the
UAS such as OASIS. - Experience providing primary care to homebound
individuals. - Care management experience for persons receiving
home and community-based services. - Home care experience as a
paraprofessional or Licensed Practical Nurse, caring for a minimum
of 2 patients per week. Description: Required licensure includes
current RN Licensure in New York State. All nurse assessors must
meet the following requirements. Preferably, candidates will have
experience conducting assessments in UAS, especially in the last
365 days. Experience caring for a geriatric population and managing
their chronic diseases in the home setting is preferred.
Combination of home care experience as a paraprofessional or LPN
along with RN experience caring for a minimum of 2 patients per
week is ideal. Experience that may count toward preferred
qualifications includes home care experience with only one patient,
case management limited to utilization reviews, and assessments
conducted telephonically only. This position requires the ability
to review health status questions and make evaluations based on UAS
criteria for enrollment. Candidates must possess knowledge of
general professional nursing theory, principles, and practices, as
well as state regulations. Excellent organizational, interpersonal,
written, and verbal communication skills are essential for this
role. Candidates should be able to communicate effectively with
multi-generational consumers and demonstrate sensitivity to their
concerns. The role requires the ability to perform comfortably in a
fast-paced, deadline-oriented environment and successfully execute
many complex tasks simultaneously. Candidates need to have a strong
ability to work independently. Job Responsibilities: Perform
initial in-home pre-evaluation of patients that includes the
evaluation of health status, strengths, care needs, and
preferences. Review consumer medical documentation and health
referral forms relevant to the case. Enter evaluation data into an
electronic evaluation form and transmit as required. Document any
concerns or conflicting information that arises during the
evaluation process. Emphasize continuity of care to reduce
fragmentation and duplication in treatment plans. Discuss with
consumers their healthcare options, supports needed, service vendor
options, and waiver options. Initiate communication with eligible
consumers for enrollment into a managed care health plan. Enhance
communication and collaborative relationships with
interdisciplinary care team members. Collect quality review data
and documentation to support outcome measurements and record case
notes. Identify opportunities for health promotion and illness
prevention. Maintain a comprehensive working knowledge of community
resources and network services for the target population. Meet all
standards established for this position as outlined in the
performance criteria. Perform other duties as assigned by the
Regional Nurse Manager or project management. All candidates must
